A Case of Listeria Monocytogenes Meningitis


Abstract
Listeriosis is originally recognized as infectious disease of domestic and wild ani¡þmals. Infection of men with Listeria monocytogenes was first described in 1929 by Nyfeldt, and in recent years it has been reported with increasing frequency. However, Listeriosis is not a common diaese.
In Korea, 12 bacteriologically proven infections were reported.
Author isolated Listeria monocytogenes from cerebrospinal fluid(CSF) of 70 year old male with esophageal carcinoma and probable liver cirrhosis, who had clinical symptoms and signs of meningitis. Laboratory studies revealed leukocyte of 19,000/ul with 30% of band and 55% of segmented neutrophil in complete blood count, and leukocyte of 713/ul with 80% of neutrophil, 72mg/dl of proteiny and 29mg/ dl of glucose in routine CSF analysis.
Under the clinical impression of bacterial meningitis, CSF culture was performed. Bacterial culture from CSF yielded tiny colonies with narrow zone of beta hemolys¡þis, of which shows typical biochemical characteristics of Listeria monocytogenes.
Susceptibility test showed this strain to be sensitive to amikacin, chloramphenicol, gentamicin and kanamycin.; and resistant to ampicillin, carbenicillin, tobramycin, cerfazolin and cefoperazone.
Treatment was started with ampicillin. Three days after admission, the treatment was changed to chloramphenicol according to susceptibility test. Six days after the onset of symptomes, the patient was afebrile and much improved the symptoms and signs.
